A certificate written in English is often needed for an employer outside Japan, a head office overseas, an insurer, a university, or a visa-related process.
Your doctor writes it directly in English. Because the doctor who examined you also writes the document, the wording can be adjusted to what the recipient actually needs to decide.
If the recipient requires Japanese, or requires both, we can do that as well.
- Please tell us who the document is for and what they need to know. A certificate written for an insurer is not the same as one written for a manager.
- If there is something you would prefer not to have written, say so before we start.

Not a translationWhy a document is not the same as a conversation
A certificate is not read for interest. Somebody reads it in order to decide something — whether to approve leave, whether to pay a claim, whether to make an adjustment at work, whether to extend a deadline.
That means the document has to answer the question that particular reader is actually asking, in the terms they use. A Japanese certificate rendered into English afterwards is a description of a different document: it will usually be accurate, and still fail to say the thing the reader needed.
Here, the doctor who examined you writes the English text as the original. Nothing is drafted in Japanese first and converted.
- Recipients often come back with a follow-up question. The doctor who wrote the document answers it, rather than the question going back through a translation step.
- If your employer's form must be completed in Japanese, we do that too — and can supply an English version alongside it so that you can read what has been written about you.

Why the feeWhy documents are charged this way
A certificate is not a form to be filled in. What has to be written changes depending on what the reader has to decide. A disability pension form, for example, asks for daily functioning to be assessed item by item and kept consistent with the clinical record.
The doctor who writes it stands behind the content, and handles any follow-up questions from the recipient. The fee covers that, including revisions.
